Summary
Purpose: This study aimed to compare the effects of transvaginal natural orifice transluminal endoscopic surgery (V-NOTES) and traditional porous laparoscopic surgery on ovarian function following the treatment of benign ovarian tumors.
Methods: The study retrospectively analyzed women who underwent ovarian cystectomy by either V-NOTES or laparoscopy at a tertiary hospital between January 2017 and October 2021.
Results: Eighty women were randomly assigned to either the laparoscopy group (n=40) or the V-NOTES group (n=40). V-NOTES showed superior control over blood loss (BL), operation time (OT), and time to first ambulation compared to laparoscopic surgery. Additionally, patients in the V-NOTES group did not require analgesics postoperatively. Following laparoscopic surgery, levels of hemoglobin (Hb), estradiol (E2), antral follicle count (AFC), peak systolic velocity (PSV), and resistance index (RI) decreased significantly, while levels of luteinizing hormone (LH) and follicle-stimulating hormone (FSH) increased. In contrast, after V-NOTES treatment, decreases in Hb, FSH, E2, PSV, and pulsatility index (PI) were observed, with no significant change in AFC and RI. Patients undergoing V-NOTES had a longer postoperative hospital stay compared to those undergoing laparoscopic surgery.
Conclusions: V-NOTES demonstrated better intraoperative and postoperative outcomes and had a smaller impact on ovarian reserve compared to laparoscopic surgery in the treatment of benign ovarian tumors.
